 PATIENT SURVEY

Mental Health and Wellbeing Matters

Henfield Medical Centre is involved in a really exciting project, the Mental Health PACE Setter scheme. It has been developed and is being led by Coastal West Sussex Mind and Coastal West Sussex CCG.

This will involve the development and training of staff in regards to mental health and wellbeing for both our staff and patients.  As a practice we would like part of this to be patient and carer focused, with help from your input concerning patient’s experience when attending the surgery and the services we offer.

With that in mind, below is a link to the PACE Setter recommended anonymous questionnaire for patients to complete. They are for everyone and not just our mental health patients.

Our Practice Profile

Henfield Medical Practice is an 8 Partnership (7 GPs and 1 Practice Manager).  It is a training and dispensing practice operating from two sites, the main surgery at Henfield and a branch surgery at Partridge Green. The practice area is mainly rural and has a growing population due to local development. We offer a range of primary care services.

We operate from well equipped, purpose built premises. Woodlawn Surgery is our branch surgery located in Partridge Green, approximately 3 miles north of Henfield.
We employ a team of nurses, including Nurse Practitioner(s) and Advanced Nurse Practitioner(s). Also Health Care Assistants offering phlebotomy and health clinics. We work closely with Community Nurses and Health Visitors.
Clinical staff are supported by a team of ancillary staff. The practice uses SystmOne clinical software and is an electronic paper-light surgery.
The Practice consistently achieves high on the Quality and Outcomes Framework, and is committed to running Enhanced Services offered by the CCG and the full range of Directly Enhanced Services.

We have an active patient participation group who have helped build a good relationship between the surgery and patients. They produce a six monthly newsletter and help patient education by arranging regular consultant talks. They also help to raise funds for a local trust set up to assist with the purchase of equipment for the surgery.
