New Patient Registration
Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.
In this respect please bring along proof of address such as a recent utility bill. Please also bring a proof of ID with your application pack.
We do not accept driving licences as proof of address.
You will also need to complete a registration form (GMS1) and a patient questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.
We use a system called GP2GP for electronic health records to be transferred directly and securely between GP practices. It improves patient care as GP's will usually have full and detailed medical records available to them for a new patient's first consultation. If you wish to know more about this please click HERE.
All new patients are offered a health check with a member of the healthcare team. Once registered you will be assigned to the doctor with the shortest list.
Medical treatment is available from the date of registration. Please contact reception for further information.
You may print off a registration form, fill it out and bring it in with you on your first visit to the practice but please ensure you sign and date all forms and bring along your proof of address and ID.
Named Accountable GP
All patients at Henfield Medical Centre are allocated a named accountable GP in accordance with our GMS contract and NHS England.
To check who your registered or accountable GP is, please ask at Reception when you are next in the surgery or you can find the information on your repeat prescription slip.
