New Patient Registration

Patients living within the practice area are welcome to register and our reception staff can confirm your eligibility from your postcode.

Due to COVID-19, we are not currently asking patients to bring in proof of address and ID. 

You will also need to complete a registration form (GMS1) and a patient questionnaire which will provide useful information whilst we wait for your medical records to arrive from your previous doctor.

We use a system called GP2GP for electronic health records to be transferred directly and securely between GP practices. It improves patient care as GP's will usually have full and detailed medical records available to them for a new patient's first consultation. If you wish to know more about this please click HERE

All new patients are offered a health check with a member of the healthcare team. Once registered you will be assigned to the doctor with the shortest list.

Medical treatment is available from the date of registration.  Please contact reception for further information.

How to register

You may print off the registration forms below, complete them as much as you can, sign all forms that ask for a signature and post them to us or scan and email them to sam.tombs@nhs.net.

If you can we would appreciate it if you could email us a copy of your ID and proof of address but due to current events, it is not necessary.

If you are unable to print or scan any of the registration forms please do call a member of staff on 01273 492255 and they can send you a registration pack in the post.

Named Accountable GP

All patients at Henfield Medical Centre are allocated a named accountable GP following our GMS contract and NHS England.

To check who your registered or accountable GP is, please ask at Reception when you are next in the surgery or you can find the information on your repeat prescription slip.
