Practice Nurse - Part-time 15 - 19 hours

Job Summary

To be responsible for the delivery of practice nursing services, working as part of the practices multidisciplinary team who provide care within their scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, well woman and well man clinics, as well as supporting the management team in the reviewing of clinical policy and procedure.

The post holder is responsible for the delivery of general practice nursing services across the whole Practice population as part of the Nursing Team. Key elements of the role are the delivery of evidence-based Practice for patients presenting with a long term conditions and the provision of preventative health care to the Practice population. They will demonstrate safe clinical decision-making and provide expert care in the management of patients. The post holder is expected to work collaboratively with all members of the Practice team to meet the needs of patients and support Practice objectives, policies and procedures. The post holder will work within their scope of practice and in accordance with local policies and procedures and the NMC Code of Professional Conduct: Standards for conduct, performance and ethics for nurses and midwives.
