Registering for Online Services

To view your test results, vaccinations, request medication and view appointments, download the NHS App.
For booking nurse appointments and viewing your medical records on the NHS App, complete the below form.

If you are requesting access to your MEDICAL RECORD:

• The process can take up to 21 days
• Record access will NOT be enabled for any individual registered with the practice for less than 3 months

If you are unable to print/email this form please collect a copy at our reception desk or call us on 01273 492255 and a member of staff will post the form to you.
